Tool Kit for Health Care Advance Planning (American Bar Association)
The American Bar Association’s Commission on Law and Aging has put together a helpful tool kit to walk you through the process of creating an advance directive. It includes how to select a healthcare agent or proxy, specific questions to help you decide under what conditions you would not want life sustaining care, how to talk to your loved ones about your desires, a structured consideration of personal priorities and spiritual values, what to do after you’ve signed your directive, and a list of resources.
